Become A Medical Marijuana Patient

The state of Colorado allows for medical marijuana under Amendment 20 of the Colorado Constitution. The process is fairly simple:

  • Patient fills out application with personal information such as address, SSN, phone #, etc. Download Application
  • A doctor fills out the physician recommendation page of the application, identifying which condition you're using medical marijuana for.
  • The application is mailed in to the State of Colorado Department of Public Health and Environment (CDPHE) via certified mail. The cost of the application is $90 (check or money order). Retain a copy for your records
  • CDPHE mails the medical marijuana card to the patient.

Quite a few doctors will not sign off on medical marijuana applications. They feel that doing so would risk their practice. If this is the case, you'll need to utilize a doctor referral service.
